TERMS AND CONDITIONS: I understand that my FJP Alumni membership serves as my acceptance of Rules and Regulations of the FJP Alumni Association. I also understand that my membership may be suspended or terminated at the discretion of FJP Alumni Management. You must be 18 years or older to join. You must be in good health and I am fully aware of and appreciate the risks, including the risk of catastrophic injury, paralysis, and even death, as well as other damages and losses associate with participation in this group activities. FEES AND CHARGES: There is an annual membership fee that is payable by you on commencement of this Agreement and is collected from you by us upon the completion of this form. Fee rate is defined in our bylaws. Events sponsored by FJP Alumni Association has separate fee rates depending on the individual campion.
